Sodium and fluid balance during exercise -

This paper provides a review of recent literature concerning the interactive effects of sodium and fluid ingestion in maintaining fluid homeostasis during and following exposure to heat and exercise.

Thus, a great deal of attention has been focused on developing fluid replacement guidelines and products for active people. Recently, there have been reports of more frequent cases of hyponatremia among individuals who tend to over-ingest water during exercise lasting more than four hours, and inclusion of sodium chloride in the fluid replacement beverage is often suggested as a potential means of reducing risk of hyponatremia.

Up to a certain point, taking in plain water is enough to mitigate sweat losses. But, as those losses start to mount up, you need to replace sodium too in order to avoid your blood becoming diluted.

This is a potentially disastrous condition called hyponatremia , which can certainly ruin your race and, tragically, has even been fatal on occasion. Having said that, figuring out whether your net losses are likely to be low, moderate, or high can be a great starting point for honing in on the level of sodium and fluid replacement that'll work best for you in different circumstances.

Calculating your sweat rate can be a bit awkward, but here's a guide to help you get you to a reasonable estimate of how much you're sweating per hour. We offer an at-rest, non-invasive Sweat Test at Sweat Test Centers around the world for anyone wanting to find out exactly how much they're losing.

Dehydration can occur when there is inadequate water levels in the body and can be very hazardous to the health of an individual. As the severity of dehydration increases, the exercise performance of an individual will begin to decline see Figure 7.

It is important to continue to consume water before, during and after exercise to avoid dehydration as much as possible. Even with the constant replenishing of water throughout an exercise, it may not be possible to drink enough water to compensate for the losses.

Dehydration occurs when the total loss of water is so significant that the total blood volume decreases which leads to the reduction of oxygen and nutrients transported to the muscle cells.

A decreased blood volume also reduces the blood flow to the skin and the production of sweat which can increase the body temperature. As a result, the risk of heat related illnesses increases. Heat cramps are one of the heat related illnesses that can occur during or after exercise.

Heat cramps are involuntary muscle spasms that usually involve the muscle being exercised, which causes an imbalance of electrolytes, usually sodium. Heat exhaustion is caused by the the loss of water decreasing the blood volume so much that it is not possible to cool the body as well as provide oxygen and nutrients to the active muscles.

Symptoms that arise from heat exhaustion may include low blood pressure, disorientation, profuse sweating, and fainting. Heat exhaustion can progress further if exercise continues into a heat stroke. A heat stroke is the most serious form of heat related illnesses that can occur.

Other symptoms that arise are dry skin, extreme confusion, and unconsciousness.

Around a third of that water exists outside your cells, in extracellular fluids like your blood. So, more sodium equals more fluid; less sodium means less fluid. Explore a fluid replacement strategy for optimal health, exercise, and sports performance for athletes. As we saw in the previous lectures, the evaporation of sweat is the major mechanism for losing heat during exercise. And this has physiological consequences because the body loses fluids. If we look at how sweating-induced dehydration affects the body fluid spaces, we can see in this slide. Other studies have suggested a progressive slight production in plasma volume over time with sweating induced dehydration.
